How To Apply For Accreditation

  • Step 1: Review the Accreditation Guide and use the PDF fillable form to check off each of the standards your agency meets. Gather evidence of how your agency meets each of the standards and prepare documents for upload in PDF format.
  • Step 2: Complete the application below using the Accreditation Guide you completed as a reference. Upload the PDF documents with evidence for each of the six standards and any optional practices you wish.  Be sure to check the attestation at the bottom of the application and click submit.  You will receive an acknowledgement that your application was submitted and receive a copy of your application in an email.
  • Step 3: Use the link at the bottom of the page to submit the required fees.
